Application Materials

Intent to Register form: Complete this form to notify us of which courses you would like to take this summer. We will try to register you for your first choice, but this is not always guaranteed. Email this form to the Office of International Initiatives at oii@gsu.edu by March 1.

Registration Agreement: Complete this form to confirm that the Office of International Initiatives has your permission to register you for Summer Institute courses. Email this form to the Office of International Initiatives at oii@gsu.edu by March 1.

Georgia State University/Summer Institute application: Complete this two-page form to officially apply to Georgia State University and the Summer Institute. Our Admissions team will process your application, and after your official acceptance, you will be able to complete your visa paperwork and pay your deposit online. Email this application to the Office of International Initiatives at oii@gsu.edu by March 1.

Georgia State University/Summer Institute application (in Mandarin): This is the same application as the English version. Please do not submit this version to the Office of International Initiatives; you will need to email us the English version. Please use this application as a reference only.

International Applicant Immigration form: Georgia State University requires immigration and financial information from each applicant who is obtaining an F-1 visa. Visit the Application Instructions page for more information about applying for an F-1 visa and for submitting your paperwork by April 1.

International Applicant Financial Information form: Use this form to accompany your International Applicant Immigration form in order to process your F-1 visa.

Certificate of Immunization: Georgia State University requires its students to have a record of immunizations before students can attend classes. Email this form to the Office of International Initiatives at oii@gsu.edu by June 1.
